Treatment of Renal Cell Carcinoma with Inferior Vena Cava Thrombu

Abstract
The prognosis for patients with renal cell carcinoma (RCC) with inferior vena cava (IVC) thrombus is very poor due to the absence of effective chemotherapeutic agents and the limited usefulness of radiation therapy in this setting. Surgery provided the only hope of a cure in these patients, although resection of RCC with IVC thrombus is technically demanding and is associated with high perioperative morbidity and mortality rates. For this reason, an aggressive approach has been established as the mainstay of treatment. Recent advances in the understanding of the biology of intravascular tumor invasion and the considerable progress in the fields of diagnosis and treatment of this entity, with innovative surgical and systemic strategies, have completely redefined the management of these patients. Basically, treatment must be individualized to every patient based on tumor biology and patient performance status. Additionally, the decision to perform invasive surgery in these patients should be done with caution.
